2008 LATE Registration
Volleyball League

I understand, although I register, it is tentative pending the approval of my parish priest and coach. I have read and understand the Rules as well as the Waiver & Assumption of Risk.

LATE     REGISTRATION      QUALIFICATIONS

I understand and AGREE that the following late registration terms must be met:
        I will pay a $50.00 late fee to “The Coptic League” in remuneration for this tardiness.
                          (The League will NOT clear a late registrant until late fee is received.)

          Please mail the check and the signed letter together to The Coptic League P.O. Box 2786 Orange, CA. 92859.  If all criteria are not met fully, no other accommodations will be made to include these
          individuals into the league this year. Thank you for your cooperation.
        I will provide a written and signed letter by my parish priest of the church, of which I will be
            playing for, to the Coptic League approving and clearing me to participate this season. 

        (This must be on church letterhead and must also state your first name, last name, and date of birth or
        the parish priest may opt to send an email noting all the same information to volleyball@lacopts.org. 
        The email must be addressed from the priest’s email address and not sent on his behalf.)

I understand that by ONLY submitting this form I am NOT allowed to participate in the league UNTIL I submit the late fees and letter to the league.
