The undersigned ("Attendee")
voluntarily makes and grants this Waiver an Assumption of Risk in
favor of The Coptic Orthodox Church, Diocese of Los Angeles and The
Coptic Orthodox Church, Diocese of Los Angeles Basketball League
(The Coptic League) as partial consideration for the opportunity to
use the facilities, equipment, materials, and/or other assets of The
Coptic Orthodox Church, Diocese of Los Angeles and The Coptic
Orthodox Church, Diocese of Los Angeles Basketball League (The
Coptic League) and/or receive assistance, training, guidance,
tutelage and/or instruction from the personnel of The Coptic
Orthodox Church, Diocese of Los Angeles and The Coptic Orthodox
Church, Diocese of Los Angeles Basketball League (The Coptic League)
and/or to engage in the activities, events, sports, festivities,
and/or gathers sponsored by The Coptic Orthodox Church, Diocese of
Los Angeles and The Coptic Orthodox Church, Diocese of Los Angeles
Basketball League (The Coptic League), I do hereby waive and release
any and all claims whether in contractor of personal injury, bodily
injury, property damage, damages, losses and/or death that may arise
from my aforementioned use or receipt, as I understand and recognize
that there are certain risks, dangers, and perils connected with
such use and/or receipt, which I hereby acknowledge have been fully
explained to me and which I fully understood in completeness, wholly
satisfactory and acceptable to me. I further agree to use my best
judgment in undertaking these activities, use and/or receipt, and to
faithfully adhere to all safety instructions and recommendations
whether oral or written. I hereby certify that I am a competent
adult or with permission of my guardian assuming these risks of my own free will, being under no
compulsion or duress.
This Waiver and Assumption Of Risk is
effective during the term of the season with The Coptic Orthodox
Church, Diocese of Los Angeles and The Coptic Orthodox Church,
Diocese of Los Angeles Basketball League (The Coptic League), and
may not be revoked, altered, amended, rescinded or void without the
expressed prior written consent of The Coptic Orthodox Church,
Diocese of Los Angeles Basketball League (The Coptic League).
Without this form you cannot engage in any of the Coptic League's
activities or functions. By initialing and submitting this waiver and assumption of
risk proves that I, the player have read and have understood the
rules. I, the player will abide by the rules and will suffer consequence(s) if I, the player break a rule. By signing,
submitting, and initialing this waiver I release and agree them to
play in The Coptic League's summer of 2007 league |
